The answers to this questionnaire will give me a really good understanding of what you would like to work through.

Please answer honestly (there is no judgment here, no right or wrong and all answers are strictly confidential).

The more of an in-depth understanding I have of what you'd like to work through the more specific I can make the prompts to enable you to truly elicit the self awareness, healing, reframes and creation you desire and deserve.

Please stick to one topic as this enables us to really dive deep. This could be a past event (or series of), present challenge, future worries, area of your life you feel dissatisfied in, feelings and habits that aren't serving you/ don't feel good etc.
